Aviva and Bupa are two of the biggest names in UK private medical insurance, and many people compare their quotes. Both are household names offering comprehensive cover and access to a wide hospital network. The choice is closer than many people expect, but the right option depends on how you want to manage costs and what matters most to you.

The core difference

Bupa's approach is inclusive by default: its comprehensive cover includes cancer and mental health benefits without extensive configuration. Aviva is designed to be flexible, with seven excess levels, the option to limit or remove outpatient cover, a guided hospital pathway at a lower cost, and a wellness discount programme. Both offer comprehensive cover, but they give you different ways to tailor your policy and manage costs.

Pricing and cost control

For equivalent cover, Aviva is consistently priced at or below Bupa, and its range of cost-control options is the widest in the UK market. If budget matters and you want to choose a major insurer, Aviva gives you more ways to reduce the cost without giving up comprehensive cover altogether.

Bupa's prices are at the higher end, reflecting its wider hospital access and more inclusive standard cover. You may pay more, but you have fewer choices to configure.

Hospital networks

Both have extensive hospital lists. Bupa's is wider, partly because it includes its own directly operated facilities, such as Bupa Cromwell Hospital in London. This difference matters most in Central London and some other cities, where Bupa's hospitals provide access to specialists who may not be available through other networks.

Outside major cities, the practical difference in day-to-day access is smaller. Both cover the main private hospital groups (Spire, Nuffield, Ramsay and Circle), and most people can find their local private hospital on either list.

Cancer cover

Both include comprehensive cancer cover on their standard comprehensive plans — surgery, chemotherapy, radiotherapy, and access to specialist cancer drugs. Aviva's cancer monitoring (via Expert Select) extends to up to 10 years post-treatment. Bupa's cancer cover explicitly includes some drugs not yet approved on the NHS. Both are among the strongest cancer propositions in the UK market.

Mental health

Aviva includes inpatient mental health treatment as standard on its comprehensive plan; outpatient mental health cover is typically an add-on. Bupa includes more mental health cover as standard on its comprehensive tier, with an optional upgrade for the most extensive provision in the market. If ongoing access to outpatient therapy matters to you, Bupa's standard comprehensive policy includes more.

Family cover

Both are strong options for families. Aviva charges a premium for the eldest child only, while covering any additional children at no extra premium. It also tracks the no-claims discount (NCD) separately for each family member. Bupa also tracks NCD per family member and offers a family discount. For larger families, Aviva's eldest-child-only pricing typically results in a lower total premium.

The NHS wait option

Both offer a six-week NHS-wait option, according to the information we reviewed. It can reduce the comprehensive plan premium by 15–25% while retaining all benefits. In return, you use NHS care when it can treat you within six weeks. Aviva's NHS Top-Up plan takes this further: the six-week wait is central to the plan, rather than an option added to a comprehensive policy.
