Health Insurance Benefit Limits: How to Check What Your Policy Pays

Published: 18 October 2026

A benefit limit is the maximum amount, number of appointments or level of service a policy provides for a particular benefit. Limits can apply to outpatient consultations, therapies, diagnostic tests or other optional cover. The label “covered” does not always mean costs are unlimited.

Find the limits that apply

Read the policy schedule and benefit table alongside the full policy wording. Check whether a limit is per person, per policy year, per condition or per course of treatment. Also check whether consultations, scans and therapies share one allowance or have separate caps.

Ask before treatment starts

Ask the insurer how much of the allowance remains, whether the provider’s fees count toward it and what happens when it is used up. Confirm if the excess applies before or after a benefit cap. Get the answer and claim reference in writing.

Can an insurer pay more than the listed limit?

Do not rely on an exception unless the insurer confirms it for your claim. Some benefits may have different terms, but only your policy can establish that.
